Five Mushroom Formula combines pure extracts of the five most commonly used medicinal mushrooms. Five Mushroom Formula is a 100% extract only formula; no powders, excipients, or fillers have been added; providing the most compact dosing possible. It is used by practitioners as a tonification therapy for serious health challenges.

Available in 100 & 250 Capsules per bottle.

As of June 2021, Five Mushroom Formula will be manufactured locally in Oregon at a third-party certified GMP factory. Each batch is independently tested in the United States for heavy metals and microbiological contaminants. (Note recent price increase)

Each capsule contains a proprietary blend of:

Coriolus extract
(Coriolus versicolor fruiting body / 15% polysaccharides) 

Cordyceps extract
(Cordyceps sinsensis mycelium / 40% polysaccharides)

Maitake extract
(Grifola frondosa fruiting body / 20% polysaccharides)

Reishi extract
(Ganoderma lucidum fruiting body / 10% polysaccharides / 4% triterpenes) 

Shiitake extract (Lentinula edodes fruiting body / 6% polysaccharides)

Other Ingredients:
Vegetarian capsules (Hypromellose)

Directions:

Take 2 capsules 2 times per day or as directed by your health-care practitioner. Higher doses may be appropriate when recommended by your health-care practitioner.
